Important: This website does not request or collect any health, diagnostic or medical record data. The booking form is for scheduling an appointment only — please do not send details about your medical condition through the website. Clinical details are discussed in person at the branch with the treating physician and stored in the medical record governed by Ministry of Health regulations.
We will not use your data for any purpose materially different from those above without obtaining your consent again.
We do not sell, rent or trade your personal data. It may be shared only in the following cases:
